Estimation of the risk of osteoporotic fractures in adult women in Argentina assessed by FRAXTM

Abstract
Aims: To estimate the risk of major osteoporotic fractures (OFR) and probability of hip fractures (HFP) assessed by FRAXTM and its association with individual risk factors considered by this method in adult Argentinean women. Methods: Cross-sectional study; consecutive sample of 146 women (40 to 75 years), attending FIEEM-ICA (Buenos Aires, Argentina 2008-2009). Dependent variables: Risk of major osteoporotic fractures (<10%; ≥10%); Probability of hip fracture (≤3%; >3%). Independent variables: Age: <60 yr and 60-75 yr. Sum of Risk Factors (RF): 0-1 RF; ≥2 RF (Personal and parental history of fracture, Smoking, Glucocorticoids, Rheumatoid arthritis, Alcohol, Secondary osteoporosis). Analysis with SPSS 15.0; OR with 95% confidence interval (CI) and p<0.05. Results: Age 60±6.3 yrs. BMI 27.6±5.0 kg/m2. Healthy weight (BMI <27.3 kg/m2) in 52.7% of the patients; none showed low BMI. OFR and HFP for most of the sample: <10% (88.4%, CI 82.8-93.9) and ≤3% (90.4%, CI 85.3-95.5) respectively. Women ≥60 yr had significantly OFR ≥10% (OR 21.52 CI= 2.77-167.25-p<0.0001) and HFP >3% (OR 16.5, CI 2.10-130.49 p<0.0001) than younger patients. The 84.2% (CI= 77.9-90.5) had 0-1 individual RF. The most prevalent were smoking: 24.7% (CI= 17.3-31.9), use of glucocorticoids: 15.8% (CI= 9.5-22.0), previous personal fracture: 14.4% (CI= 8.3-20.4) and parental fractures: 13.7% (CI= 7.8-19.6). There was significant association between OFR or HFP, and previous personal fracture (p <0.0001), and glucocorticoid use (p <0.01). Conclusions: Age, previous personal fractures and use of glucocorticoids were significantly associated with OFR and HFP.
